Dr. James J. DiResta's letter (from PM Pulse, 8.14.26) regarding the socialization, or not, of the American health system struck a chord with me, but maybe not the one he was after. The future of American medical care and how best to distribute it to the public has engendered much debate over many years and in many forums.

IL Podiatry Group Urges Medicare Advocacy
CMS recently published the proposed rule for the Calendar Year 2027 Medicare Physician Fee Schedule, which included a proposed 50 percent cut for services reported on the same day with Modifier 25. The Illinois Association of Podiatric Physicians and Surgeons (ILAPPS), working with the APMA, is urging its members to “make your voice heard today and tell your members of Congress and CMS directly to withdraw this proposal!” Click below to read more about this proposal and learn how to get involved in your own state.
Is Private Equity Changing Its Tune on Podiatry?
Over a dozen states now have laws enhancing their oversight of private equity deals in health care. A new report says that’s putting a crimp in dealmaking.
Physician practice management is on track to see about half the number of deals this year as it did in 2025. Investments in the businesses, which run clinics’ billing and operations, have plummeted from a high of 851 deals in 2021 to just 105 in the first half of 2026, according to new data from PitchBook.

The Need to Focus on Positive Engagement Around Podiatry Professional Issues
If you really want to do this right, I’m genuinely glad you decided to break away from the old format. For years, there has been far too much negativity surrounding podiatry in those discussions, often driven by individuals who have not actively practiced in decades or who no longer represent the profession in a meaningful way. Unfortunately, that type of commentary creates a distorted picture of what podiatry is today and where it is headed.
What we need is a platform that highlights the many positive developments occurring throughout the profession. There are countless examples of podiatrists taking on leadership roles within hospitals and health systems, becoming department chairs, serving in executive positions, and helping shape the future of patient care. The profession has grown significantly, and opportunities continue to expand in both academic and private practice settings.

Cybersecurity Gaps That Could Cost You
4. Cut off access the hour someone leaves
The former employee whose login still works is one of the most common and most preventable security gaps. When a staff member departs, a contractor's engagement ends or a vendor relationship closes, access needs to end at the same time, not at the end of the week. The proposed HIPAA update sets an aggressive benchmark: terminate access within one hour of a workforce change.
Build a written offboarding checklist that covers every system, EHR, email, billing, portals, building access and any shared passwords, and assign one person to run it. Review active accounts on a regular schedule to catch the ones that slipped through. Orphaned accounts are especially dangerous because no one is watching them, which makes them a quiet favorite of attackers.

How to Cut Patient No-Shows
11. Track your no-show data so you fix the real driver
You cannot manage what you do not measure. Calculate your no-show rate by dividing missed visits, including late cancellations, by total appointments, then break it down by provider, specialty, appointment type and even day of the week. The patterns are rarely uniform, and the averages hide where the real problem lives.
That detail is what turns effort into results. If new-patient visits drive most of your misses, front-load reminders and confirmation there. If a particular provider or slot runs hot, the fix may be scheduling rather than messaging. Watch the numbers as you roll out changes so you can tell what is actually working. Every fix on this list gets sharper when you point it at the specific place your no-shows are coming from.
