Re: Looking for Answers: L3000 Denials
What would be helpful is knowing which insurance plan these denials are coming from. Have you communicated with them and asked them why suddenly this happened?
More than likely this is a computer glitch. The reason, because the Medically Unncessary Edits (MUE) set a limit on how many units or times a HCPCS code can be billed on the same date. However, there are exceptions, such as for Left or Right. And since billing pairs of a HCPCS unit should not, according to HIPAA billing standards, be billed on the same line, you are submitting the claims correctly.
However, the exception the MUE may not have been encoded or may have been stripped out of the insurer's system. Hence the denials.

Re: Looking for Answers: L3000 Denials
Sorry for the confusion with my initial response. There was a typo. My bad!
With respect to the initial explanation where the "NU" modifier is present, please eliminate the "NU" modifier. The NU modifier is defined as “New Equipment.” This modifier is not needed to refile the claim.
Once again, with respect to the Fix:
Leave Line 1 alone. On Line 2 (L3000-LT), append modifier XS (Separate Structure) or modifier 59 (Distinct Procedural Service).
The Sequence:
Line 1: (Already Paid): L3000-RT
Line 2 (The Correction): L3000-LT, XS (or L3000-LT, 59)

Marketing Metrics That Matter
When it comes to collecting and analyzing data on your practice, the sky’s the limit. Everything can be collected and analyzed. How many patients came through the door? Done. Number of new patients per day, week, month, year? Done. Revenue per patient? Done.
If you have the kind of mind that craves more and more data, almost anything is up for grabs. But with limited time and resources, it makes much more sense to focus on specific metrics that support your goals.
This article will focus on the marketing metrics that will drive growth in your practice, which excludes many other business metrics that can and should be analyzed, but for different reasons.

APMA Submits Comments on ICD-10-CM Improvements
APMA submitted comments to the National Center for Health Statistics at the Centers for Disease Control and Prevention regarding improvements to the ICD-10-CM coding system for diabetic foot ulcers. The comments support more accurate coding and documentation for diabetic foot ulcers to better reflect the care podiatric physicians provide. This advocacy arose from work the Wound Care Collaborative Community has done with support from APMA.
