Today’s podiatric physicians are continually searching for ways to differentiate themselves from other providers in an increasingly competitive healthcare environment. One of the most effective strategies is the development of a niche practice. And among the most rewarding and sustainable niches in podiatry is a practice that’s focused on biomechanics and sports medicine.

I do believe a critical factor is getting training in sports medicine for the provider., not finding ways to market to attract patients. The article mentions staff training but not for the practitioner. The perpetuation of "biomechanics" being orthoses fabrication is undervaluing the training needed to be a true sports medicine expert. Residency does not prepare most graduates to treat high school, let alone professional athletes and there are few sports medicine fellowships for podiatrists…

An ambient tool captures the conversation. It does not capture the silent physical exam, the procedure performed without narration, the reading on a device in the corner or the clinical reasoning a physician never says out loud. Those gaps show up in the chart as missing documentation, and missing documentation shows up later as a downcoded claim or a failed audit.

Two fixes work. Train clinicians to narrate the elements that matter, which most find awkward for about a week and natural after that. And build a short post-visit checklist for the pieces a scribe reliably misses, particularly the medical decision-making and time elements that payers scrutinize. A note that reads beautifully and supports a lower level of service than the visit warranted is still a revenue problem.

MGMA submitted comments to the Centers for Medicare & Medicaid Services (CMS) in response to Medicare enrollment proposals included in the Calendar Year 2027 Home Health Prospective Payment System (HH PPS) Rate Update proposed rule. MGMA urged CMS to withdraw sweeping Medicare enrollment proposals as their cumulative effect would be significant and detrimental to medical group operations. In addition to our individual comment letter, MGMA joined other provider organizations in a joint letter urging CMS to reconsider the proposed changes.
