The security risk analysis is the one control regulators cite in nearly every enforcement action, and it is where many practices fall short. A real analysis is not a checkbox or a template filed away and forgotten. It is an honest inventory of where ePHI lives, how it moves and where it is exposed, refreshed at least once a year and after any major change. The proposed rule would make the annual cadence explicit, along with a written technology asset inventory and network map.

Start by listing every system and device that stores or touches patient data, then rank the risks and document what you are doing about each one. The documentation matters as much as the work: if a breach happens, a current, thorough risk analysis is your strongest evidence that you took security seriously.
