When billing BCBS of Texas for custom-molded foot orthotics using L3000-RT and L3000-LT, the C-04 and N-519 rejection triggers specifically because of how the bilateral lines are formatted or combined on the claim layout.
CO-4 (Claim Adjustment Reason Code): The procedure code is inconsistent with the modifier used, or a required modifier is missing.
N-519 (Remittance Advice Remark Code): Invalid combination of HCPCS modifiers.
Together, they mean BCBSTX rejected the claim line because the modifiers appended to the CPT/HCPCS code are either incompatible with each other, incorrect for that specific procedure code, or required but missing entirely. The system flags an "invalid combination of HCPCS modifiers" (N-519) because the billing system expects the anatomical modifiers to be submitted under very specific structural criteria.
How to Fix the Second Line:
Since the first line is already paid, do not resubmit the whole claim as new, or you risk a "duplicate claim" rejection for the right foot.Submit a Corrected Claim with a "Distinct Structure" Modifier. You need to tell the BCBSTX claim processing system that the second line is a distinct piece of equipment for a completely different foot.
The Fix:
Leave line 1 alone. On Line 2 (L3000-LT), append modifier XS (Separate Structure) or modifier 59 (Distinct Procedural Service).
The Sequence:
Line 1 (Already Paid): L3000-RT
Line 2 (The Correction): L3000-NU-LT-XS (or L3000-NU-LT-59)
Submission:
File this as a Corrected Claim (Frequency Code 7), reference the original claim number, and state in the remarks: "Resubmitting line 2 with modifier XS (or 59) to resolve modifier conflict on bilateral custom orthotics."
Match the Box 22 Requirements:
Because you are using Frequency Code 7, you must link this new submission to the original processing cycle so the adjuster knows it is a correction:
Claim Frequency Code: Set to 7 (Replacement).
Original Reference Number (ICN/CCN): Input the exact claim number from the BCBSTX Remittance Advice (EOB) that paid the first line and denied the second. '
Format the Claim Lines Exactly:
To bypass the C-04 / N-519 duplicate edit filter, structure your lines to clearly state the left foot is a distinct structure.
Line 1: L3000-RT 1 unit (Leave exactly as originally billed (matches what they paid)
Line 2: L3000-RT, XS (or RT, 59) 1 unit (Modified line to bypass the rejection)
Add a Clear Statement in the Remarks/Notes:
In Box 19 (or the electronic equivalent loops for claim remarks), add a brief note for the claims processor: "Corrected claim submitted to add modifier XS (or modifier 59) to Line 2 (LT side) to resolve automated modifier/duplicate conflict. Line 1 (RT side) was previously processed and approved."
Once submitted, BCBSTX should pull the original claim out of their system, replace it with this version, and should process the payment for the left foot.
This is my opinion.
